This is accomplished by integrating the skills and knowledge of our team members, which consists of over 50 years' experience in the waiver world and educational foundations in psychology, human services, social work and business management.

Case Management

Case management is the collective gathering of assessments, planning, facilitation, care coordination and advocacy for services and opportunities to meet a participant’s community-based needs through communication, person-centered planning and matching of available resources to promote independence, safety, quality of care and economical outcomes.

Case management services are best delivered when direct communication between the case manager, the client, and provider, is consistent and honest to optimize an independent community-based setting for the participant.
