The Community Choice Waiver (CCW) is a program available to adults 19 – 64 who have a disability verified by the Social Security Administration and adults who are 65 years and older. The program provides a menu of services that allow those who qualify to stay in their communities.
Your case manager will assist in ensuring a smooth application process.
Once funding has been approved, your case manager will assist in developing a plan of care by discussing needs and services that can be fulfilled with waiver funds, interviewing, and touring providers and guiding the self-direction process if the family determines that it best fills the needs of the participant.
The participant and/or their family will have contact with their case manager monthly at minimum to ensure satisfaction with their plan of care, chosen services and providers.
